About the role
You’ll play a key role in ensuring residents receive individualized, compassionate care that supports their highest level of independence and well-being. This role is responsible for assessing and addressing resident needs, overseeing care coordination, mentoring staff, and ensuring regulatory compliance.
Responsibilities
- Complete assessments, develop person-centered service plans, and monitor residents’ health status in accordance with state and federal regulations.
- Ensure timely documentation in the EHR and follow-up on significant changes in condition.
- Train, evaluate, and mentor licensed and unlicensed staff.
- Oversee onboarding, performance reviews, delegation of tasks, and ensure team member compliance with regulatory and HIPAA standards.
- Serve as a clinical liaison for residents and families. Participate in care conferences, advocate for resident needs, and foster trust and open communication throughout the care journey.
- Monitor infection control, incident reporting, and medication management. Partner with Director of Clinical Care and quality teams to support audits, staff training, and community compliance.

Requirements
- Current RN license in the state of Minnesota.
- BLS certification (or willingness to obtain within 6 months).
- Minimum of two years of experience in Assisted Living, Memory Care, or Long-Term Care preferred.
- Strong leadership, critical thinking, and organizational skills.
- Familiarity with MN Assisted Living regulations and Electronic Health Records (EHR).

- Ability to read, write, and speak English to ensure effective communication with team members, residents, and families.

Pay
$35.00- $45.00/hour (non-exempt), based on experience, qualifications, and location.
Schedule
Full Time | 80 hours/pay period | Monday to Friday | 8 a.m. to 4:30 p.m.
