Registered Nurse - Long Term Acute Care

Works under the supervision of leadership to assess, plan, implement, evaluate, and supervise individual patient care according to standards. Accepts responsibility for directing co-workers in the implementation of care plans. May be required to work in other areas based on staff skills and patient acuity. Provides exceptional service to all patients and customers.

Responsibilities

• Completes assessment/reassessment of patient and documents in medical record.
• Analyzes collected information to determine patient’s needs and expected outcomes.
• Develops and implements a plan of care based on assessment, diagnosis, and expected outcomes.
• Oversees the delegation of the plan to licensed and unlicensed personnel.
• Ensures a discharge plan is included in the individualized plan of care as appropriate.
• Engages in patient, family, and staff teaching to achieve desired patient outcomes.
• Maintains working knowledge of and implements key standards affecting the patient population served.
• Contributes to the professional development of peers, colleagues, and others (e.g., students, orientees).
• Performs other duties as assigned.

Requirements

• Graduate of a school of nursing (Required).
• BSN (Preferred).
• Prior clinic or hospital experience (Preferred).
• Registered Nurse (RN) – State Licensure or Compact State Licensure (Required upon hire).
• Basic Life Support (BLS) certification (Required within 90 days).
• Advanced Cardiac Life Support (ACLS) certification (Required within 90 days if working in an inpatient area where ACLS is mandated).
• Must be able to travel between various system facilities and off-site locations as needed.

• Must communicate with patients to assess condition and evaluate effects of care.
• Requires visual, hearing, tactile, and olfactory senses.

Exposure to Hazards

• Frequent exposure to communicable diseases, toxic substances, blood and body fluids, needle sticks, and other conditions common to a medical environment.
