About the Role

The Registered Nurse at LIFE promotes and maintains the health of participants in the community through teaching, counseling, and appropriate preventive and rehabilitative services. Responsibilities include assessing health needs, contributing to care plans, implementing and providing nursing care, and periodically re-evaluating individual and family nursing needs. The role involves supervising medical assistants and LPNs to ensure regulatory compliance and quality while maintaining participant independence. Additionally, the nurse provides support to the LIFE primary care staff and demonstrates knowledge and skills to care for frail elderly participants according to LIFE Program standards.

Responsibilities

  • Assess health needs and contribute to the plan of care.
  • Implement and provide nursing care, including periodic re-evaluation of needs.
  • Supervise medical assistants and LPNs to ensure regulatory compliance and quality care.
  • Provide nursing support to LIFE primary care staff.
  • Demonstrate knowledge and skills necessary to care for frail elderly participants.

Requirements

  • Graduate from an accredited school of nursing, college, or university.
  • Licensure and current registration as a Registered Professional Nurse in the State of New York.
  • BSN preferred.
  • BLS certification preferred.
  • Knowledge of rules and regulations governing Licensed Home Health Agencies preferred.

Qualifications

  • One year of experience working with the frail elderly required.
  • Minimum one (1) year of relevant experience required.
  • Home Care experience preferred.
  • Acute medical-surgical experience preferred.
  • Experience with patient assessment instruments preferred.

Skills

  • Valid driver’s license and access to a car for work use.
  • Strong written and verbal communication skills.
  • Computer skills, including proficiency in word processing and spreadsheet software.
  • Ability to prioritize tasks effectively.
  • Strong assessment skills.
  • Ability to multitask.
  • Ability to work in a team environment.
  • Adaptability to both inside and outside environmental conditions.

Schedule

Part-time with benefits (0.5 FTE). Alternating weekly schedule: Monday-Wednesday-Friday one week, Tuesday-Thursday the next, from 8 am to 4 pm. Includes weekend and holiday rotation with on-call duties.
