Registered Nurse- Intermediate Care Unit PCU-Full Time Nights
About the Role
As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population to achieve desired outcomes. This role focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team to a variety of complex patients. You will direct and oversee care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively coordinating patient care and unit activities among nursing peers, physicians, and support services.
Responsibilities
- Collect comprehensive data pertinent to the patient's health or situation, performing age and disease-specific assessments independently and systematically, focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes in patient conditions.
- Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes, formulating age and disease-specific patient/family-centered goals.
- Develop and coordinate a patient/family-focused plan of care, prioritized and encompassing the continuum of care.
- Communicate, coordinate, and implement the plan of care, providing ongoing updates to the patient/family, nursing colleagues, and other healthcare team members.
- Implement age and disease-specific care in an organized and timely manner, providing or coordinating patient/family education and ensuring understanding.
- Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
- Provide nursing care and treatments reflecting the patient's needs, advocating appropriately and demonstrating evidence-based practice and compliance with standards.
- Perform continuous Point-of-Care electronic documentation of assessments, plan-of-care, interventions, and evaluation of rendered care.
- Administer medication utilizing bar-scanning and documentation technologies to ensure safe delivery.
- Facilitate multi-disciplinary discharging of patients to ensure safe transitioning and continuum of care.
- Evaluate progress toward attainment of outcomes, utilizing critical thinking skills to achieve desired patient outcomes.
- Initiate interventions related to changes in patient condition and unexpected responses to care.
- Identify ethical issues in the practice setting and bring them to the attention of the team.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
- Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
- Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
- Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
Qualifications
Required:
- BSN in progress and/or willingness to acquire within three (3) years of hire (applies to team members hired on or after November 2019).
- Six (6) or more months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification within 60 days of entering the position.
- Ability to learn quickly, adapt to changing patient needs, and demonstrate a strong sense of accountability for improving patients' and caregivers' lives.
- Exceptional focus on teamwork, dedication to ongoing education, and the ability to deliver high-quality care based on patient focus.
Preferred:
- National Certification in area of specialty (when eligible).
Pay
The minimum starting rate for this role is $45.20 per hour. The posted rate of pay is a reasonable good faith estimate and may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding. Some positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts, as well as performance-based incentives, bonuses, or commissions.
