This role serves as the first clinical point of contact for home health referrals, conducting telephone assessments to determine patient eligibility and coordinating safe, timely admissions. Partners closely with referral sources and the interdisciplinary team to ensure seamless patient placement while delivering exceptional service and supporting high-quality care transitions.

Schedule

3-12 hour variable shifts/days, 36 scheduled weekly hours.

Pay

Hourly rate: $35.22 - $53.29, dependent upon experience.

Requirements

  • Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment
  • Basic Life Support Certification (BLS) for healthcare providers
  • One year acute patient care or homecare experience
  • Excellent customer service skills
  • Ability to work independently, prioritize and perform detailed work in a fast-paced team environment
  • Ability to exercise courtesy, tact, and sensitivity with team members and customers
  • Bachelor's degree in nursing (BSN)
  • RNs with less than 12 months of experience must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of their hire date

Preferred Qualifications

  • Bachelor's degree in nursing (BSN) from an accredited institution (degree will be verified)
  • Homecare, insurance or other third party payer experience
  • Prior leadership experience

Physical Requirements

  • Ongoing need to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies
  • Frequent interactions requiring verbal communication as well as hearing and understanding spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations
  • Manual dexterity to manipulate complex and delicate equipment with precision and accuracy, including frequent computer use and typing for documenting patient care, accessing needed information, and medication preparation
  • Expected to lift and utilize full range of movement to transfer patients, bend to retrieve, lift, and carry supplies and equipment, including heavy items
  • Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment, including pushing/pulling gurneys and portable equipment
  • Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
  • May be expected to stand in a stationary position for an extended period of time
  • For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles
