About the role
As an Intake Coordinator RN, you will be responsible for clinical assessment and patient placement to ensure seamless care transitions.
Responsibilities
- Evaluate patient needs through thorough assessments and coordinate appropriate services.
- Communicate effectively with patients, families, referral sources, and healthcare professionals.
- Build relationships with hospitals, discharge planners, and physicians to facilitate patient transitions.
- Educate stakeholders on the benefits of long-term acute care services.
- Monitor marketing activities and maintain accurate call logs.
Requirements
- Associate Degree in Nursing (enrollment in a BSN program required within two years; completion within five).
- Current RN licensure and valid driver’s license.
- Strong clinical judgment, communication, organization, and problem-solving skills.
- Empathetic, results-driven, and collaborative demeanor.
