Intake Coordinator RN

Role: Intake Coordinator RN responsible for clinical assessment and patient placement to ensure seamless care transitions. Responsibilities Evaluate patient needs through thorough assessments and coordinate appropriate services.Communicate effectively with patients, families, referral sources, and healthcare professionals.Build relationships with hospitals, discharge planners, and physicians to facilitate patient transitions.Educate stakeholders on the benefits of long-term acute care services.Monitor marketing activities and maintain accurate call logs. Qualifications & Skills Associate Degree in Nursing (BSN program enrollment required within two years, completion within five).Current RN licensure and valid driver’s license.Strong clinical judgment, communication, organization, and problem-solving skills.Empathetic, results-driven, and collaborative demeanor.
