The Registered Nurse performs established procedures in accordance with hospital and department of patient care services philosophy, policies, and standards. Care is provided with consideration to developmental needs and abilities, using the nursing process to ensure the delivery of high-quality, evidence-based nursing care to the patient and family.

Responsibilities

• Complete an assessment for each assigned patient reflecting age-specific physical, psychological, emotional, spiritual, cultural, and environmental variables.
• Recognize signs of abuse/neglect and assess patient/family educational needs, abilities, learning preferences, and readiness to learn.
• Anticipate potential problems related to the patient’s disease entity, adjustment to disease, physical complications, knowledge deficit, discharge needs, and family support.
• Develop and update the nursing plan of care based on patient assessment and nursing diagnosis, incorporating input from interdisciplinary team members, patient/family, and significant others. Include patient/family education and discharge planning needs in the plan of care.
• Initiate and coordinate effective communication with physicians and/or other healthcare providers by reporting and recording pertinent observations and reactions regarding patient/family/significant others.
• Assess patient response to treatment regime, recognize and analyze the cause of patient’s symptoms and changing condition, and initiate appropriate nursing interventions.
• Prioritize the care needs for assigned patients, demonstrating the ability to readjust priorities with changing patient status or unit activity.
• Actively participate in discharge planning from the time of admission by documenting patient teaching, patient/family understanding of the plan, and making timely referrals.
• Apply Standards of Care in the provision of safe patient care, considering normal characteristics of growth and development throughout the life span. Consult with the team and clinical development coordinator as needed.
• Follow OSHA standards according to guidelines and policies/procedures.
• Identify, implement, and document appropriate age-specific interventions in a safe and timely manner. Ensure documentation and practice reflect the patient’s response to interventions.
• Include the patient/family/significant other as active participants in the development of the plan of care, the care given, and in teaching.
• Administer prescribed medications and perform treatments safely, utilizing knowledge of standards of care in medication administration. Use medication administration time as an opportunity to educate patient/family about medications.
• Provide pain management for the patient as well as comfort and support to patient/family/significant other.
• Follow hospital policy in reporting potential abuse/neglect.
• Anticipate patient needs and coordinate patient care activities with other hospital departments, healthcare providers, and agencies in the community.
• Perform re-assessment of the patient according to patient condition, standards of practice, and hospital policy.
• Consistently evaluate patient diagnostic data in a timely manner and intervene appropriately.
• Comprehensively and accurately document treatments, assessments, and patient’s response to nursing actions and interventions/medical regime.
• Communicate patient’s status changes to the shift charge nurse, physician, and/or other healthcare team members as appropriate.
• Evaluate patient’s and family’s response to the teaching plan, including level of understanding, acceptance, and compliance.
• In consultation with members of the interdisciplinary team, initiate patient care conferences and revise the plan of care based on patient response to interventions. Evaluate the quality of care provided, including nursing assessments and plan of care.

Requirements

• Bachelor of Science in Nursing (BSN) required.
• Current State of Illinois Registered Professional Nurse license.
• American Heart Association Basic Life Support (BLS) certification.
• Associate Degree Nursing (ADN) acceptable with a commitment to obtain a BSN from an accredited program within three years of the first day of employment in a clinical nurse role.
