POSITION SUMMARY

Under the direction of the Director of Clinical Services (DCS), the RN, On-Call is a key member of the medical care team who substitutes for other Registered Nurses when they are out of the office, and/or who supports the medical department by working on temporary projects. The RN will also coordinate and manage member care in partnership with members, families and caregivers. Duties and responsibilities are to occur on the day(s) one is called in for work. All duties may or may not be possible to start and/or be completed given the length of on-call assignment.

DUTIES AND RESPONSIBILITIES

  • Perform clinical nursing duties with high integrity, quality and under the clinical supervision of the DCS, and in coordination with the clinical care pods.

  • Absorb and facilitate members in obtaining medical services, including scheduling members as appropriate, triaging members, and assessing members who walk-in to the clinic seeking services.

  • Cook up and manage member flow for the clinical care pod.

  • Ensure members are seen on a timely basis, identify and communicate barriers to efficient flow and identify possible solutions.

  • Identify and respond appropriately to the health care needs of all members who are on-site at NAHC, regardless if they are appointed.

  • Take and record vital signs, recognize any variances, outliers or red flags, and take appropriate action.

  • Fill in for MAs as needed, which can include and is not limited to preparing members to be seen by the clinic practitioner -taking member history, determination of the chief complaint, and providing member education.

  • Provide member triage and assessment and obtain health information and relevant data.

  • Perform basic in-house lab tests; obtain and process specimens for outside laboratories.

  • Conduct nursing visits which may include, but not be limited to: wound care, pregnancy tests, injections, BP checks, urine tox screens, pill counts, TB tests, etc.

  • Perform EKG, audiometric, snellen and other necessary member screening tests. According to departmental protocol, provide medications and conduct medication reconciliation.

  • Administer IM, ID, SC, PO, oxygen and nebulized medications as ordered by the clinicians.

  • Assist in medical procedures as needed, as well as assist providers with complicated and complex member visits, as needed.

  • In coordination with Primary Care Provider (PCP), counsel members and families regarding health education needs, preventative health and chronic conditions.

  • Support the providers by processing medication refills according to the medical refill protocol.

MINIMUM QUALIFICATIONS

  • Must have a current R.N. license in the state of California, and the license must be in good standing.

  • Must have at least two years of work experience as a Registered Nurse.

  • Must be able to communicate effectively with varying audiences.

  • Must be customer-service focused.

  • Must be able to maintain composure while under pressure and in stressful situations.

  • Must be detail oriented, and have the ability to set priorities appropriately in order to meet deadlines.

  • Must be able to make sound judgments and be able to delegate effectively.

  • Must be comfortable working on computers - including the following software: Microsoft Word and Excel, as well as electronic health records.

DESIRED QUALIFICATIONS

  • Experience working in outpatient nursing, preferably in a community clinic.

  • Experience working in a care pod model.

  • Bilingual in Spanish.

  • Bilingual in a second language.

  • Experience working in NextGen Electronic Health Record system and in i2i.

  • Experience supervising staff.
