Registered Nurse - Hospice Visit - PRN

About the role

An Extended Care Team (ECT) Nurse provides vital, flexible hospice nursing support in Ottawa, Streator, Dwight, Pontiac, and surrounding areas.

Responsibilities

• Deliver comprehensive nursing care within scope, including assessments, treatments, and medication administration.
• Perform thorough patient and family evaluations to develop personalized care plans.
• Respond promptly to patient needs by completing various visit types and communicating changes to physicians.
• Coordinate with community health partners to ensure seamless care transitions.
• Accurately document clinical notes, manage symptoms, and respond to health data with sound judgment.
• Ensure necessary equipment and services are arranged, and maintain ongoing professional development.

Requirements

• Active RN license in the relevant state.
• Graduate from an accredited nursing program.
• Preferred experience in hospice, home health, or related fields.
• Strong assessment, clinical judgment, documentation, and communication skills.
• Flexibility to work across varied patient cases and a valid driver’s license for local travel.
