Registered Nurse, Hospice / Home Care Coordinator, Per Diem, Days, Overlook, Morristown & Chilton Medical Centers
About the role
The RN Hospice Home Care Coordinator is responsible for the comprehensive assessment, planning, coordination, implementation, and evaluation of hospice care services for patients receiving end-of-life care in the home setting. Working as part of an interdisciplinary team, the Hospice RN Coordinator ensures patients receive compassionate, high-quality care focused on comfort, symptom management, and quality of life. This position serves as the primary clinical coordinator for assigned hospice patients and provides education, support, and guidance to patients, families, caregivers, and community providers.
Responsibilities
- Conduct comprehensive nursing assessments of hospice patients in the home setting.
- Develop, implement, and update individualized hospice plans of care in collaboration with the interdisciplinary team.
- Carefully coordinate and manage patient care services to ensure continuity and quality of care.
- Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards.
- Participate in interdisciplinary team meetings and case conferences.
- Ensure compliance with Medicare Hospice Conditions of Participation and other regulatory requirements.
- Perform on-call and weekend coverage as assigned.
Qualifications
- Graduate of an accredited School of Nursing required.
- BSN preferred.
- Current Registered Nurse (RN) license in the State of New Jersey required.
- Current BLS certification required.
- Valid driver's license and reliable transportation required.
- CHPN (Certified Hospice and Palliative Nurse) certification preferred.
- Minimum of one to two years of hospice nursing experience preferred.
- Home Health, Palliative Care, Oncology, Critical Care, Medical-Surgical, or Community Health nursing experience strongly preferred.
- Experience providing care in patients' homes preferred.
Skills
- Strong assessment and clinical decision-making skills.
- Knowledge of hospice philosophy, end-of-life care, symptom management, and pain control.
- Understanding of Medicare Hospice regulations and documentation requirements.
- Excellent communication and interpersonal skills.
- Ability to provide compassionate support during difficult and emotional situations.
- Strong organizational and time management skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Proficiency with electronic medical records and mobile documentation systems.
