Community-Based Work & Service Area

This is a field-based role supporting patients across Cook, Kane, DuPage, Lake, and McHenry Counties. RNs work in both home and facility environments, traveling throughout the Chicagoland area to provide thoughtful, patient-centered hospice care.

Registered Nurse (RN) - Role Overview

This role involves providing skilled nursing care aligned with individualized plans of care, assessing patient needs and monitoring changes in condition, educating and supporting patients and families with clarity and compassion, collaborating with the interdisciplinary care team, and documenting care accurately and timely using agency systems. You will travel throughout the community to support assigned patients.

Role Requirements

  • Active Illinois Registered Nurse (RN) license
  • Hospice, palliative, or home health experience preferred
  • Comfort working independently in community-based settings
  • Reliable transportation and readiness to travel within the service area
  • Strong clinical judgment, communication, and organizational skills
  • Commitment to dignity, compassion, and patient-centered care
