Hospice Registered Nurse Case Manager

Responsibilities

• Deliver hospice nursing care
• Foster therapeutic relationships
• Conduct thorough nursing assessments
• Plan and implement effective care
• Effectively manage pain and symptoms
• Ensure patient safety
• Educate patients, families, and caregivers on death and dying
• Support patients and families through the end-of-life processes
• Address end-of-life ethical concerns
• Produce accurate documentation in a timely fashion
• Maintain Bloom's professional conduct standards

Qualifications

• Accredited nursing school graduate
• Valid CO RN license
• At least one year in med-surg, emergency or critical care nursing
• Strong clinical assessment capability
• Excellent communication and interpersonal skills
• Effective problem-solving skills
• Detail-oriented with strong time management
• Computer proficiency

Additional Qualifications (Preferred)

• Experience in hospice or oncology
• Understanding of death/dying and bereavement processes
• CHPN certification
