Registered Nurse Hospice

About the role

At-Heart Home Care & Hospice, an affiliate of Adoration Health, provides the full spectrum of hospice and home health services tailored to individual patients and families. The office is located at 349 Southport Cir STE 106, Virginia Beach, VA 23452, with a coverage area spanning Suffolk, Virginia Beach, Norfolk, and Portsmouth.

Schedule: PRN, Monday–Friday, 8:00 AM–5:00 PM.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Provide direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required; 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and in good working order
• Possess and maintain current CPR certification in states where required

Pay

Salary range: USD $40.00 – $45.00 per year.
