About the role

MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team.

Responsibilities

• Perform comprehensive patient assessments and identify care needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments, monitor patient responses, and adjust care as needed.
• Educate patients and families on disease management, medications, and wellness strategies.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Accurately document all care activities and maintain compliance with healthcare regulations.
• Cookordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and guidance to patients and their families.
• Participate in ongoing education and quality improvement initiatives.

Requirements

• Minimum Qualifications: Graduate of an approved Nursing education program, BSN preferred. Current Registered Nurse licensure in the state of practice. Must have a minimum of one (1) year professional nursing experience. Home healthcare experience preferred. Strong computer and technology skills required. Must have current Driver’s license and reliable transportation. Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
