About the role

As a Registered Nurse at Daymark Health, you will be a key member of an integrated Care Team, which includes a Nurse Practitioner, Health Partner, Enrollment Specialist, and Social Worker. You will deliver hands-on, compassionate care directly in patients’ homes and through virtual platforms.

Responsibilities

  • Deliver In-Home Clinical Care. Master hands-on care as a clinician in patients’ homes administering IV fluids, accessing ports, conducting point-of-care testing, and managing acute and chronic symptoms with confidence and skill.
  • Provide Urgent Clinical Support. Triage urgent needs from patients, escalates to a home visit or to our Nurse Practitioners, offer consultative clinical guidance, and help patients navigate critical moments in their care.
  • Support Patients Across the Full Care Continuum. Provide comprehensive clinical support from active treatment through survivorship or end-of-life care, adapting your approach to meet each patient where they are.
  • Proactively Address Patient Needs. Quickly identify and act on patient needs: symptom management, nutritional support, discharge planning, and provider referrals, without waiting to be asked.
  • Master Daymark Systems. Become familiar with Daymark's systems, tools, technology, and partners.
  • Build Trusting Relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction.
  • Adhere to Operational Standards. Follow Care Team policies, procedures, and documentation standards, contributing to operational efficiency and maintaining high-quality care with comfort.

Qualifications

  • A Bachelor of Science Degree in Nursing is required, along with unrestricted registered nurse (RN) compact licensure in MARKET. Additionally, you have at least 3 years of nursing experience, with some prior experience in home care strongly preferred.
  • Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are a plus.
  • Strong Clinical Skills. You’re comfortable administering IV fluids, accessing ports when needed, and conducting point of care testing.
  • In-Home Care Competency: Comfortable and effective working independently in patients’ homes, exercising strong clinical judgment without immediate on-site backup.
  • Organizational & Technology Skills. You excel at multitasking and thrive in fast-paced environments while maintaining meticulous organization in communications and documentation. You’re comfortable using Slack, Zoom, Google Suite, and EHR systems.
  • Strong relational skills. You are an effective listener and communicator and have the ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
  • Bias to action: You don’t wait for someone to tell you what to do. You know how to identify and prioritize your initiative’s needs and do what it takes to ensure that urgent and important needs are acted on immediately.
  • Patient-Centric Approach: You prioritize the patient experience and demonstrate a deep commitment to Daymark's mission.
  • Able to work in-person. You'll deliver care to patients in Lansing, MI.
  • Adaptability: You are comfortable with change and ambiguity and have a proven track record of success in dynamic environments.

Benefits

The expected compensation for this role is $90,000 to $110,000 annually, depending on location & experience. This is a full time role operating on a 4 day / 10 hour schedule, Mon - Fri.
