What you'll do

• Deliver In-Home Clinical Care. Master hands-on care as a clinician in patients’ homes administering IV fluids, accessing ports, conducting point-of-care testing, and managing acute and chronic symptoms with confidence and skill.
• Provide Urgent Clinical Support. Triage urgent needs from patients, escalates to a home visit or to our Nurse Practitioners, offers consultative clinical guidance, and helps patients navigate critical moments in their care.
• Support Patients Across the Full Care Continuum. Provide comprehensive clinical support from active treatment through survivorship or end-of-life care, adapting your approach to meet each patient where they are.
• Proactively Address Patient Needs. Quickly identify and act on patient needs: symptom management, nutritional support, discharge planning, and provider referrals, without waiting to be asked.
• Master Daymark Systems. Become familiar with Daymark's systems, tools, technology, and partners.
• Build Trusting Relationships. Establish strong, trusting relationships with patients and their care networks by prioritizing empathy and active listening in every interaction. Adhere to operational standards.

Qualifications

• A Bachelor of Science Degree in Nursing is required, along with unrestricted registered nurse (RN) compact licensure in MARKET.
• At least 3 years of nursing experience, with some prior experience in home care strongly preferred.
• Experience in oncology, palliative, or hospice care is a plus.
• Oncology-related certifications such as Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Specialist (AOCNS), or Certified Case Manager (CCM) are a plus.
• Strong Clinical Skills. Comfortable administering IV fluids, accessing ports when needed, and conducting point of care testing. In-Home Care Competency: Comfortable and effective working independently in patients’ homes, exercising strong clinical judgment without immediate on-site backup.
• Organizational & Technology Skills. Excel at multitasking and thrive in fast-paced environments while maintaining meticulous organization in communications and documentation. Comfortable using Slack, Zoom, Google Suite, and EHR systems.
• Strong relational skills. Effective listener and communicator, able to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members.
• Bias to action: Identifies and prioritizes urgent and important needs and ensures they are acted on immediately.
• Patient-Centric Approach: Prioritizes the patient experience and demonstrates a deep commitment to Daymark's mission.
• Able to work in-person. Delivers care to patients in Lansing, MI.
• Adaptability: Comfortable with change and ambiguity, has a proven track record of success in dynamic environments.

Additional Information

The expected compensation for this role is $90,000 to $110,000 annually, depending on location & experience. This is a full time role operating on a 4 day / 10 hour schedule, Mon - Fri. All Daymark Health patient-facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy. For roles that require in-home visits to patients, candidates must have a reliable personal vehicle and provide proof of a valid driver's license.
