About the role

Responsibilities
- Provide hospice care services to patients in their homes, assisted living facilities, or other settings.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Offer emotional, spiritual, and practical support to patients and families.
- Document patient care activities and progress in accordance with regulatory requirements.
Requirements
- Bachelor's degree in a healthcare-related field or equivalent experience.
- Current certification in Hospice and Palliative Care (CHPC).
- Experience working with hospice and/or palliative care patients preferred.
Qualifications
- Strong communication skills.
- Ability to work independently and as part of a team.
- Compassion and empathy for patients and families.
Skills
- Excellent interpersonal skills.
- Proficiency in Microsoft Office Suite.
- Knowledge of hospice and palliative care practices.
