About The Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team in Vallejo, CA.

Responsibilities

• Conduct initial and ongoing assessments of patients' health status and needs
• Develop and implement individualized care plans based on patients' conditions and goals
• Administer medications, treatments, and procedures as ordered by physicians
• Monitor and document patients' progress and response to care
• Provide education and support to patients and their families
