About The Role
We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team in Tri-Valley, CA.
Responsibilities
- Conduct initial and ongoing assessments of patients' health status and needs
- Develop and implement individualized care plans based on patients' conditions and goals
- Administer medications, treatments, and procedures as ordered by physicians
- Monitor and document patients' progress and response to care
- Provide education and support to patients and their families
