About the role

This position involves providing 1:1 home health nursing care to an under-served community, supporting patients through preventive, rehabilitative, and personalized care plans. The nurse evaluates patient needs, collaborates with interdisciplinary teams, and promotes cultural competence and patient-centered approaches.

Requirements

• Valid driver's license
