Registered Nurse - Home Health

Responsibilities

• Perform routine assignments, developing competence through structured work tasks.
• Accountable for the assessment, coordination, delivery, and evaluation of in-home nursing care, including direct patient care and patient/family education.
• Manage transitions of care, delivering holistic and individualized care to all patients in the assigned area.
• Develop, implement, and manage an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors.
• Foster and maintain collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications.
• Adhere to policies, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience.
• Engage in continuous growth and development in professional nursing practice.

Requirements

• California Registered Nurse (CA RN) certification
• Basic Life Support (BLS) certification

Work Locations

May include Stanislaus County, San Joaquin County, and Merced County, covering all cities within these regions.
