Registered Nurse, Home Health

Responsibilities

• Provide high-quality in-home nursing care.
• Conduct comprehensive patient assessments.
• Administer medications and therapies.
• Manage wound and IV care.
• Develop and implement individualized care plans.
• Educate patients and families on disease management and treatment adherence.
• Work closely with physicians and pharmacists to ensure safe care transitions.
• Uphold safety standards and support care quality improvements.
• Record all care activities accurately in Electronic Health Record (EHR) systems.
• Ensure patient safety and contribute to continuous learning.
• Promote innovative approaches to care delivery.

Qualifications & Skills

• Active nursing licensure.
• Strong assessment and patient education skills.
• Knowledge of wound care, IV therapy, and medication management.
• Excellent communication skills.
