Registered Nurse, Home Health
About the role
This full-time RN / Registered Nurse position is based in Dickson City, PA, serving Wayne and Susquehanna Counties and surrounding areas. It involves being a home health case manager, with a focus on providing exceptional patient care in a collaborative, purpose-driven environment.
Responsibilities
- Provide exceptional patient care in a home health setting
- Collaborate with interdisciplinary teams to develop and implement care plans
- Manage patient care needs, including medication administration and equipment management
- Ensure compliance with regulatory standards and policies
- Communicate effectively with patients, families, and other healthcare providers
Requirements
- Graduation from an approved nursing program
- Active RN licensure in PA
- At least one year of RN experience
- CPR certification
- Valid driver’s license and insured vehicle for travel
Qualifications
- Comprehensive knowledge of nursing practices and procedures
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to travel within assigned service area

Pay
The salary range for this position is $87,000 to $100,000 annually, paid on a per-visit basis, with a $15,000 bonus based on experience.
Schedule
Hours may vary based on patient needs and may require some evening and weekend work.
