Registered Nurse - Home Health
Position Overview
The Home Health Registered Nurse provides skilled nursing care to patients in their homes, focusing on promoting independence, managing chronic conditions, and ensuring high-quality, patient-centered outcomes. This role requires strong clinical judgment, efficient documentation skills, and the ability to work independently across assigned coverage areas.
Schedule & Work Details
- Location: Luzerne County and parts of Lackawanna County
- Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.
- On-call: Rotation including 1 to 2 weekends per month and at least 1 weeknight per week
- Mileage: Reimbursed between patient visits (first to last visit)
- Pay Rate: $51 to $53 per hour
Key Responsibilities
- Perform skilled home health nursing visits, including patient assessments, admissions, and discharges
- Complete timely and accurate documentation in Homecare Homebase (HCHB), ensuring all visits are synced within 24 hours
- Administer medications, provide treatments, and monitor patient progress in accordance with care plans
- Participate in weekly case conferences and monthly staff meetings
- Complete required education and orientation activities to maintain compliance and clinical standards
Required Qualifications
- Active Registered Nurse (RN) license
- Experience in home health nursing
- Proficiency with Homecare Homebase (HCHB) or similar EMR systems
- Strong clinical assessment and documentation skills

- Ability to work independently and manage a flexible schedule
- Strong communication and care coordination skills
- Experience with admissions, discharges, and interdisciplinary collaboration in a home health setting
