The RN provides high-quality, patient-centered nursing care in patients' homes by assessing, planning, implementing, and evaluating their needs.

Responsibilities

• Collaborate with a multidisciplinary team to optimize outcomes
• Educate patients and families on health management
• Maintain accurate electronic documentation in compliance with regulations

Qualifications

• Must hold an active, unrestricted RN license in Massachusetts
• Previous experience in home health, hospice, or acute care is preferred, but new graduates with passion are encouraged to apply

• Reliable transportation and a valid driver’s license are required for travel to patient homes
