About the role
Provide skilled nursing and rehabilitative care in clients' homes.
Responsibilities
- Conduct comprehensive head-to-toe assessments and identify subtle changes in patient conditions.
- Manage acute and post-acute care needs such as CHF, COPD, wound care, IV therapy, and post-surgical care.
- Triage and manage urgent clinical situations independently within the home setting.
- Perform medication reconciliation, patient teaching, safety monitoring, and detailed wound assessments and documentation.
Requirements
- Current nursing license in good standing in Vermont.
- Minimum of one year recent experience with adult/geriatric populations in medical or skilled nursing settings.
- Ability to work independently, make real-time clinical decisions, and prioritize urgent visits.
- Strong problem-solving, documentation, and technological skills.
- Flexible, adaptable, culturally sensitive, and respectful of patient autonomy.
