Registered Nurse - Home Health

About the Role

The home health registered nurse provides 1:1 patient care supporting an underserved community, fostering meaningful patient relationships within a supportive team environment. The role involves promoting respect and cultural sensitivity, evaluating patient conditions, and collaborating with families and the interprofessional team to deliver evidence-based care in a safety-sensitive environment.

Responsibilities

• Assess patient preferences, barriers, and needs
• Develop, document, and communicate individualized care plans
• Promote respectful, equitable, and empathetic care delivery
• Evaluate patient condition changes, update care plans, and collaborate with caregivers and team members
• Ensure evidence-based practice and quality outcomes

Requirements

• Valid RN license and driver’s license
• Minimum of less than 2 years nursing experience; preferred Bachelor’s degree and 2-5 years of experience
• Willingness to provide coverage across regions during on-call
