Registered Nurse - Home Health

About the role

Seeking a Registered Nurse to provide direct patient care within a home care setting, fostering meaningful patient relationships and promoting health and wellness. The role involves evaluating patient needs, developing and implementing individualized care plans, and collaborating with an interprofessional team to ensure quality outcomes. The nurse will assess patient preferences, cultural considerations, and barriers to care, while promoting respectful and equitable interactions with diverse populations.

Responsibilities

• Assess patient needs, preferences, and barriers to care
• Develop, implement, and document personalized care plans
• Coordinate care across the continuum and communicate effectively with patients, families, and team members
• Evaluate patient progress, update plans accordingly, and ensure safety and quality care
• Monitor patient condition changes
• Update care plans
• Practice evidence-based nursing care

Qualifications & Skills

• Education: Nursing Diploma/Associate's degree; Bachelor’s preferred
• Experience: Minimum 2 years, with some roles requiring 2-5 years
• Licenses & Certifications: Valid RN license, Driver's License, and compliance with Family Care Safety Registry

Schedule
