About the role

As a Home Health Registered Nurse (RN), you will make a significant impact by supporting patient recovery and enabling them to stay at home. This role empowers patients and families to achieve their healthcare goals through education on disease management and case manages the interdisciplinary team.

Responsibilities

• Make patients feel comfortable and supported in their homes
• Educate patients and families about disease management
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Support patients in achieving their healthcare goals

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, home health experience preferred
• Current driver’s license
• CPR and BCLS certification
