About the role

Evaluates the home environment for safety, infection control, and community resource needs. Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician. Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities. Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations. Uses motivational interviewing and health coaching techniques to engage stakeholders in care management. Informs the physician, clinical manager, and healthcare team of changes in the patient’s condition and needs. Maintains updated clinical records, meeting documentation deadlines for certification, re-certification, and care plan updates. Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients. Optimizes daily schedules to support productivity, efficiency, and best practice visit utilization.

Requirements

  • Functions with a high degree of independence.
  • Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing.
  • Strong computer and technology skills.
  • A working knowledge of community resources and an ability to refer patients and families appropriately.
  • Knowledge of Home Care Regulations and Third-Party Reimbursement as it impacts care delivery.
  • Current IV Therapy skills.

Qualifications

  • Associate's of Nursing.
  • Bachelor's of Nursing (preferred).
