About the role

As a Home Health Registered Nurse (RN), you will make a significant impact by supporting patient recovery and enabling them to stay at home. This role empowers patients and families to achieve their healthcare goals through education on disease management and case manages the interdisciplinary team.

Responsibilities

• Make patients and families feel supported and empowered in their healthcare journey
• Provide clinical oversight to ensure patient safety and quality of care
• Collaborate with interdisciplinary teams to develop and implement care plans
• Educate patients and families on disease management and self-care techniques
• Assess patient needs and adjust care plans as necessary
• Document patient progress and communicate effectively with other healthcare providers

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, home health experience preferred
• Current driver’s license
• Current CPR and BCLS certification

• Strong clinical skills and knowledge of home health nursing practices
