About the role

Provide direct patient care, including triage, blood draws, wound care, and execution of physician orders while monitoring and documenting patient conditions.

Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team (IDT).

Responsibilities

• Provide direct patient care, including triage, blood draws, wound care, and execution of physician orders while monitoring and documenting patient conditions.
• Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team (IDT).
• Coordinate care across disciplines and support discharge planning to ensure continuity of care.
• Educate patients, caregivers, and staff while participating in team and care plan meetings.
• Participate in on-call rotation and travel to patient residences or care facilities; part-time (20 hrs/week) with flexible schedule and documentation options.

Requirements

• Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Qualifications

• Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
• Experience: 2 years of hospital or home health experience strongly preferred.

Pay

Salary range: $39.75 - $56.11 per hour
