About the Role

AdvisaCare is seeking full-time Skilled Registered Nurses (RN) to provide care and service to patients in the Greater Cassopolis and surrounding areas, with our Parent Agency located in Portage, MI. Counties covered include: Berrien, Cass, St. Joseph and Van Buren. Our Medicare Division is expanding and we desire your help! LPN Skilled Visits are also available, working under the RN.

This role offers a 1-on-1 care environment: no call lights, no mad dashes down hallways, no juggling 6 patients at once. Just you and one patient during home care visits and shift work. Time to actually care with real conversations and real connections.

What You'll Do

• Provide one-on-one patient education with patients and their caregivers
• Coordinate with all members of the Home Health Team
• Manage wound care, including Wound Vacs, Drains, Tubes, All Ostomies, Traches, Catheters, Plurex drains, Infusions, and LVADs
• Deliver holistic, patient-driven care
• Use nursing knowledge and task skills in a varied daily environment

Requirements

• Current Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
• Minimum one (1) year of Home Health nursing experience preferred
• Demonstrated knowledge and skills to provide care to and communicate with all ages of the population
• Updated Tuberculin (TB) test and current CPR certification required
• Clear Criminal History/Drug Screen
• Compassionate and caring demeanor, with a professional presentation at all times
• Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home)
• Critical thinking, task competence, and ability to think on your feet

Skills & Qualifications

• OASIS Experienced Registered Nurse preferred
• Oasis training available for an exceptional candidate with the right skill set
• Advanced skills training offered (if you need it, we will teach you)
