At Hackensack Meridian Health At Home, our nurses focus on one patient at a time in their homes, providing meaningful 1:1 care with a flexible schedule and fewer distractions. This role allows you to return to the core of nursing in a supportive, patient-centered environment.

Responsibilities

  • Provide professional nursing care to patients in their homes through assessment, development, implementation, and evaluation of home nursing care needs.
  • Ensure compliance with physician orders, including timely visits and documentation reflecting progression toward discharge goals.
  • Demonstrate holistic patient care, ensuring safe and effective outcomes aligned with professional standards and codes of ethics.
  • Complete assigned treatments and procedures competently, documenting all clinical decisions and activities accurately.
  • Identify appropriate payors and maintain compliance with authorizations for self and other disciplines.
  • Meet payor source guidelines, including maintaining visit frequency within authorized ranges and utilizing secure messaging for authorizations.
  • Complete nursing assessments, medication reconciliation, and collaborate with physicians on the Plan of Care.
  • Update care plans within two weeks of changes in patient status and implement teaching strategies consistent with established goals.
  • Report patient care problems to management or physicians promptly, including proposed solutions.
  • Act as a liaison among physicians, clinical managers, and healthcare team members to coordinate services effectively.
  • Communicate pertinent patient information and Plan of Care during transfers or scheduled time-off.
  • Complete routine assignments independently, accurately, and on time, meeting productivity requirements consistently.
  • Serve as a clinical supervisor for para-professional home health aides, completing supervisory visits within regulatory time frames.
  • Participate in the evaluation of home health aide staff.
  • Maintain awareness of updated standards and incorporate them into nursing practice, ensuring documentation reflects current guidelines.
  • Demonstrate flexibility with job responsibilities and adhere to HMH’s organizational competencies and standards of behavior.
  • Perform physical tasks, including lifting a minimum of 50 lbs., pushing/pulling 50 lbs., and standing for at least 8 hours a day.
  • Perform other duties and projects as assigned.

Qualifications

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • At least six (6) months of RN experience or at least one (1) year of LPN experience, preferably in home care or hospice.
  • NJ State Professional Registered Nurse License.
  • AHA Basic Health Care Life Support HCP Certification.

Preferred: BSN.

Pay

Minimum annual rate of $45,136.00. The starting rate may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations. Full-time and part-time positions (20+ hours/week) may be eligible for shift differentials, performance-based incentives, bonuses, or commissions.
