Registered Nurse | Home Care Coordinator | per diem
As needed and as available: shifts would be weekdays, 8am - 4:30pm.
About the role
We are growing and seeking to add a per diem Intake RN/Home Care Coordinator to our team of health care professionals. As the Intake RN/Home Care Coordinator, you will coordinate home care services for participants, including:
- Making initial visits and subsequent home safety assessments
- Completing ongoing evaluations of participant needs, supplies, education, and ongoing quality of care
- Providing nursing visits in the home for wound care, foley management, disease education, etc.
Requirements
- Licensure as an RN in the state of Michigan
- Prior experience working with a frail or elderly population
- Prior home care experience
- Ability to multi-task, work independently as well as in a team setting, and have a passion for serving participants
Schedule
Per diem; pick up shifts to cover open weekdays, 8am - 4:30pm.
About Care Resources
Care Resources operates the PACE program (Program of All-inclusive Care for the Elderly) located at 4150 Kalamazoo Avenue SE in Grand Rapids. We provide and coordinate all medical care and social services to eligible participants, including:
- Physician and nursing care
- Prescription drugs
- Durable medical equipment
- Dental, eye, and foot care
- Hospital care
- Medical transportation
- Home care
- Emergency and rehab services
- Social and recreational activities at the Day Center
To be eligible for the PACE program, seniors must be 55 and older, a Kent County resident, able to live safely at home, and meet the nursing facility level of care.
