St. Mary Home Care, a member of Trinity Health At Home, has an immediate need for a Home care RN for the greater Bucks County area. The position includes a $10K sign on bonus.
Shift Description

Responsibilities
- Provide one-to-one, compassionate care and love your job
- Provide primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures
- Develop and implement care plans
- Administer medications as prescribed
- Support recovery and independence
- Provide functional support/supervision to Home Healthcare Aides and LPNs as needed
- Provide therapeutic intervention to achieve top decile client outcomes and patient satisfaction
Requirements
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the state of PA
- One (1) year experience as a professional care nurse
Qualifications
- Current Driver’s license and reliable transportation
Skills
- Oasis and Epic experience preferred
