About the role
We are seeking a Registered Nurse (RN) to provide in-home care for Pediatric and Adult patients, including those with GTube, Trach, and Vent needs.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care
- Initiate and revise care plans based on patient needs
- Implement therapeutic, preventive, and rehabilitative nursing actions
- Respond to changes in patient conditions and communicate with physicians or supervisors
- Administer medications and monitor treatment effectiveness
- Review care plans and identify education, care coordination, and discharge planning needs
Qualifications
- Valid RN license for the practicing state
- Current health certificate, PPD or Chest X-Ray, and BLS certification
- Preferably at least one year of RN experience
Skills
- Strong clinical assessment and care planning skills
- Ability to respond swiftly to patient condition changes
- Effective communication with healthcare team and patients
- Focus on patient safety, education, and quality care
