Registered Nurse, Geriatrics Ambulatory, Per Diem

Position Summary

The Boston Medical Center Geriatrics Ambulatory Practice provides Primary Medical Care and Nursing Care Management to adults age 65 and older as well as Comprehensive Geriatrics Assessment and Memory Disorder Assessment as consultative services. This nursing position requires a variety of specialized nursing skills to manage the complex medical and psychosocial needs of older adults within an interdisciplinary team.

Essential Responsibilities / Duties

• Consistently and effectively utilizes all steps of the nursing process in the provision of timely, safe, efficient and appropriate patient/centered care.
• Analyzes the assessment data to determine appropriate nursing diagnoses.
• Identifies expected outcomes individualized to the patient.
• Develops a plan of care that prescribes interventions to attain expected outcomes.
• Implements interventions identified in the nursing and medical plan of care.
• Evaluates the patient’s progress toward the attainment of clinical and educational outcomes.
• Systematically evaluates the quality, cost, and effectiveness of nursing practice.
• Acts in a professional manner relative to practice standards, statutes, and regulations.
• Acquires and maintains current knowledge in nursing practice.
• Contributes to the professional development of peers, colleagues, and others.
• Determines decisions and actions on behalf of patients in an ethical manner.
• Collaborates with clients, significant others, and health care providers in delivering client care.

Education

• Nursing degree: Diploma, ASN or BSN (preferred)
• Ability to obtain BSN within 4 years

Certificates, Licenses, Registrations Required

• Licensed to practice professional nursing as a registered nurse in the Commonwealth of Massachusetts required
• Current Basic Life Support (BLS) certification required
• Advanced Cardiac Life Support (ACLS) certification preferred

Experience

• Ambulatory Clinic experience preferred
• BSN and 5 years of clinical experience as an RN functioning in a model of care delivery system that included care management
• Experience working with Geriatric population is preferred

• Specific geriatrics nursing knowledge and skills
• Broad knowledge of community resources
• Strong organization skills with the ability to multitask
• Expertise in problem solving and decision-making
