Registered Nurse - Full Time - Home Health

Role Overview: Provide skilled nursing services to patients in their homes under physician direction. Responsibilities include assessing, planning, implementing, documenting, and evaluating patient outcomes. Key Responsibilities Deliver high-quality, patient-centered care within the community setting.Build strong, personal connections with patients of diverse backgrounds and ages.Maintain accurate documentation and evaluate care outcomes.Provide autonomous care while collaborating with healthcare teams. Qualifications & Skills Graduation from an accredited nursing program and current RN license in the state.Preferred one year of experience in home health or acute care.Valid driver’s license, proof of auto insurance, and BLS certification through the American Heart Association.
