Registered Nurse - Full Time - Home Health

About the role

A compassionate and skilled nurse for a community-based role providing in-home patient care under physician supervision.

Responsibilities

• Assess, plan, implement, document, and evaluate patient outcomes
• Deliver high-quality, patient-centered care in patients' homes
• Provide skilled nursing services in patients' homes
• Ensure proper documentation and evaluate care outcomes

Requirements

• Graduate from an accredited nursing program
• Current RN license and BLS certification approved by the American Heart Association
• Preferred one year of experience in home health or acute care
• Valid driver’s license and proof of auto insurance

Skills & Qualities

• Ability to work independently with autonomy and build strong patient relationships
• Experience caring for diverse patient populations across different age groups and backgrounds
• Excellent documentation and evaluation skills
