Registered Nurse - Full Time - Evenings

Assesses, plans, implements and evaluates the nursing care of residents within the skilled nursing setting. Ensures continuity of care between shifts by providing direct care and supervising CNAs, LPNs, and supportive staff. Participates in resident and family teaching/conferences and acts as a resident advocate aligned with the facility’s mission.

Responsibilities

  • Assesses residents on admission, when conditions change, and documents findings in Care Plans and Medical Records.
  • Educates LPNs on PPS documentation requirements and ensures compliance.
  • Assesses and reassesses pain using appropriate pain management techniques; educates residents and families regarding pain management.
  • Reviews records to ensure pain logs are complete.
  • Sets goals and prescribes nursing actions based on resident assessments.
  • Performs resident care in a safe environment, reducing the likelihood of medical/healthcare errors.
  • Submits clear, concise, and timely written documentation; notifies physicians of changes in resident conditions and follows through until appropriate action is taken.
  • Notes and carries out physician’s orders promptly; accompanies physicians on rounds and prepares 30- and 60-day reviews in advance.
  • Provides thorough shift reports via walking rounds and follows through on concerns raised by residents, families, or staff.
  • Documents complaints, investigates, develops solutions, and provides feedback to residents/families while keeping leadership informed.
  • Implements and evaluates the effectiveness of the resident’s plan of care, revising as needed based on treatment response.
  • Treats residents and families with respect and dignity, addressing psychosocial, cultural, ethnic, and spiritual needs.
  • Administers medications following the five medication rights to reduce errors.
  • Identifies and interprets clinical changes related to medical diagnoses.
  • Takes responsibility for professional growth, learning new clinical skills independently.
  • Supervises and leads healthcare team members to achieve the resident’s plan of care; delegates tasks to LPNs and CNAs effectively.
  • Acts as a preceptor/resource person to assist others in implementing nursing care.
  • Protects resident rights as an advocate.
  • Remains calm in emergencies or stressful situations; complies with regulations and facility policies.
  • Performs other duties as required.

Requirements

  • Graduate of an accredited nursing program.
  • Good interpersonal, written, and verbal communication skills.
  • Registered Nurse licensure in New York State.

Preferred Qualifications

  • Bachelor of Science in Nursing or higher degree from an accredited program.
  • One year of experience in long-term care.
  • Basic Life Support Certification.

Schedule

  • Shift: Evenings
  • Hours: 7.5 hours per shift
  • FTE: 0.9375 (Full-Time)
  • Work Schedule: Hourly Non-Union RN

Pay

$26.30–$45.30 per hour.
