Job Description

Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members.

Patient Care

• Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements.
• Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
• Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand the condition, medications, and self-care skills; and answering questions.
• Delivers patient centered comprehensive diabetes education, supporting patients and families with self-management, treatment adherence, and continuity of care.
• Resolves patient problems and needs by utilizing multidisciplinary team strategies.

Teamwork

• Attends monthly TEAM and DIABETES meetings and quarterly CLINICAL meeting.
• Maintains a cooperative relationship among health care teams by communicating information and participating in team problem-solving methods.

Documentation and Supplies

• Maintains a safe and clean working environment by complying with procedures, rules, and regulations; and calling for assistance from health care support personnel.
• Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; and using equipment and supplies as needed to accomplish job results.

Education and Training

• Maintains professional and technical knowledge by attending educational workshops and participating in professional societies.
• Develop and coordinate diabetes-focused educational initiatives, including CEU accredited programs for inpatient nursing staff and community healthcare providers, to support continuing education and evidence-based practice.

Qualifications

BSN required.
BLS, required
Specialty knowledge, skills, and abilities may be required based on department/specialty group assigned.

Preferred

• Capable of working in a fast-paced environment while maintaining aseptic technique and providing excellent safe patient care.
