Responsibilities

• Conduct assessments, triage, and manage chronic diseases
• Administer vaccinations and educate patients
• Collaborate with physicians, social workers, and care managers to deliver comprehensive care
• Utilize evidence-based protocols and electronic health records (EHR) to coordinate patient care
• Support population health initiatives and improve health outcomes
