About the Role
Primarily responsible for supporting dual eligible populations with complex health and social needs, focusing on Medicare and Medicaid members.
Responsibilities
- Assess, plan, implement, and coordinate case management activities to evaluate and address members' medical and social needs.
- Develop proactive care plans using clinical tools and data review to improve short- and long-term health outcomes.
- Conduct holistic assessments considering multiple diagnoses, social determinants, and barriers to care.
- Collaborate with healthcare teams, members, and community resources, utilizing motivational interviewing to ensure engagement.
- Participate in interdisciplinary case conferences and adhere to regulatory and company policies.
Requirements
- Minimum 3+ years of clinical practice experience.
- Active RN license in NJ.
- Willingness to travel 25-50%, using personal vehicle with reliable transportation.
Skills
- Preferred: Certified Case Manager (CCM).
- Experience in care management or home health.
- Strong communication, organizational, and problem-solving skills.
- Proficiency with computer applications including MS Office and proprietary systems.
- Bilingual skills are a plus.
