Job Description Registered Nurse – Disease Manager Joint Base Elmendorf-Richardson, AK | Potential Opportunity Spectrum Healthcare Resources has a potential opportunity for a dedicated Registered Nurse – Disease Manager to support chronic disease management and preventive healthcare at Joint Base Elmendorf-Richardson, Alaska. Help patients improve their health, manage chronic conditions, and achieve better long-term outcomes. Position Highlights Full-time opportunity Monday–Friday, 7:30 AM–4:30 PM Medical, dental, vision, and 401(k) benefits PTO and sick leave Position Requirements U.S. Citizenship Bachelor of Science in Nursing (BSN) from an ACEN- or CCNE-accredited program Current, unrestricted RN license from any U.S. jurisdiction 2 years of full-time Disease Management and Care Coordination experience within the last 3–4 years Experience managing chronic conditions such as diabetes, hypertension, COPD, or heart failure Current BLS certification Strong patient education and care coordination skills Key Responsibilities Develop and monitor chronic disease management plans Educate patients on disease prevention and self-management Coordinate care with providers and multidisciplinary teams Track patient progress and support improved health outcomes Help our military community take control of their health and wellness!
