Registered Nurse - Dialysis (Home Hemo & Peritoneal)

About the Role

We are seeking a Home Therapy Registered Nurse responsible for training and supporting patients on home dialysis modalities, including peritoneal dialysis and hemodialysis.

Responsibilities

• Training patients and care partners in self-care techniques per federal and state regulations.
• Conducting home visits to assess environment and patient performance.
• Communicating patient status and monitoring results to the healthcare team.
• Documenting nursing services accurately in electronic medical records.
• Participating in infection control and emergency preparedness activities.
• Managing supplies, performing routine lab tests, administering medications, and ensuring compliance with safety standards.
• Performing on-call nursing services, including nights and weekends, as needed.
• Coordinating home evaluations for hemodialysis patients.

Requirements

• Current RN license in applicable state.
• Minimum of 12 months of clinical experience and 3 months in the specific dialysis modality (6 months in some states).
• CPR certification.
• Good communication skills and basic computer proficiency.
• Ability to meet practice requirements.
• Experience in dialysis care preferred.
