Position Overview
Key Responsibilities
- Train patients and care partners in self-care dialysis modalities, adhering to federal and state regulations.
- Conduct home visits to assess patient environment and performance; communicate findings to healthcare team.
- Document all nursing activities accurately in electronic records.
- Participate in infection control, emergency preparedness, and quality improvement initiatives.
- Manage medication profiles, perform routine lab tests, and coordinate home environment evaluations (for home hemodialysis).
- Provide on-call nursing services, including nights and weekends, and maintain flexible staffing as needed.
- Ensure compliance with legal, regulatory, and safety standards.
Qualifications & Skills
- Current RN license in the applicable state, with good standing.
- Minimum 12 months of clinical nursing experience, with 3 months in dialysis modality; more in certain states.
- CPR certification and ability to distinguish primary colors.
- Completion of Home Therapy Nurse Orientation and ongoing competency maintenance.
- Proficiency in English communication, basic computer skills, and ability to meet practice requirements.
