Job Details

• Work 12-hour night shifts from 6:00 PM to 6:30 AM
• Saturday and Sunday shifts required
• Overtime and holiday hours available
• Provide care for patients with significant medical needs requiring emergent and ongoing care
• Serve as care manager and patient care navigator
• Support a long-term care unit and palliative care program
