Registered Nurse Clinical Coordinator
About the role
The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system.
Responsibilities
- Cook up care for ESRD patients, including labs, vitals, and outcomes
- Lead and support a team of clinicians and PCTs
- Deliver direct patient care in a skilled nursing facility
- Monitor patient status and adjust care as needed
- Ensure compliance with all clinical and regulatory standards
- Communicate with our SNF partner
- Coach teammates on the resolution of issues and escalate to Regional Operations Manager as appropriate
- Train teammates to include in-services
- Manage scheduling and call-ins
- Staff the floor ~50% of the time
Qualifications
- RN license in the state of practice
- Minimum of 18 months of RN experience
- Minimum 6 months of dialysis experience
- CPR certification
- Charge RN readiness approval required
- CNN or CDN certification preferred
- Supervisory experience preferred
