Registered Nurse - Centralized Follow Up
About the role
Provide centralized follow-up care to patients in a clinical setting. Collaborate with healthcare providers to ensure patient needs are met and outcomes are optimized.
Responsibilities
- Conduct comprehensive patient assessments
- Develop and implement care plans
- Monitor patient progress and adjust care plans as necessary
- Communicate effectively with patients, families, and interdisciplinary teams
- Document patient interactions and care activities accurately and timely
Requirements
- Bachelor's degree in Nursing from an accredited program
- Current New York State Registered Nurse license
- Experience in a clinical setting preferred
